MEDICAL CASE MANAGEMENT

Medical case management is a consumer-driven service that links the consumer with health and social services. It is the coordination of access to appropriate medical treatment and support services, and the assurance of continuity of care. Medical case managers advocate on behalf of their clients, and educate them in order to empower each person to act on his/her own behalf. Medical case management activities also include initial and on-going assessments of the consumer’s needs; development of a comprehensive and individualized service plan; coordination of and linkage to needed services; and monitoring of the plan to assess its efficacy. Medical case management is the vital support service that assures a responsive and consumer-driven system.

PEER SUPPORT SERVICES

Peer Mentors are available to provide hands-on support and encouragement to new consumers entering the system. Since Peer Mentors have experienced the system and mastered it, they can assist and accompany the new consumer through the maze of available services. All services are available in English and Spanish.

FOOD PANTRY

The Food Pantry was established to provide nutritional food on an emergency basis. Food baskets are designed to contain well-balanced foods from the five major food groups.

AICP: AIDS INSURANCE CONTINUATION PROGRAM

AICP is a program available to help consumers with their insurance premiums for private health plans for which they may be eligible. Hope & Help Center is the only site for this service in the Central Florida area.

PHARMACY ELIGIBILITY SCREENING

For consumers who require financial support to obtain their medications, Hope & Help Center provides screening for pharmacy services at BioScrip Pharmacy. Consumers are screened to assure their financial eligibility for pharmaceuticals through available funding sources. Compassion programs through individual pharmaceutical companies are also explored.

EDUCATION/OUTREACH

Hope and Help Center has operated its award-winning HIV/AIDS/STI Prevention Program since 1989. The program features extensive education and outreach efforts throughout Central Florida.  Hope & Help Center staff is available to area businesses for HIV/AIDS education and certification courses.


MPOWERMENT

Men Uniting Men (MUM) is a dynamic community of young gay and bi-sexual men promoting HIV prevention and social change. The program conducts weekly educational groups and outreach events. This program, located at 1918 Woodward Street, Downtown Orlando, is sponsored in part, by a grant from the Florida Department of Health.

SISTA

Sisters Informing Sisters on Topics about AIDS was developed by African American women for African American women in response to increasing rates of HIV in this group. This program includes discussions and role playing scenarios, as well as the use of artwork and poetry to educate women, increase self esteem and give them the tools for maintaining their sexual health. Hope and Help is collaborating with the Sanford Housing Authority and the Seminole County Jail to bring this 5 session program to areas of Central Florida where African American women are in particular risk for contacting HIV. This program is sponsored in part by a grant from the Blue Foundation.

Project CLEAR

Project Community Level Education for Amphetamine Risk-Reduction is our newest program. With the collaboration of Orange County Government, our goal is to empower members of the community to stop (or never start) using crystal meth, and embrace healthy lifestyle choices.
