PREPARING FOR YOUR VISIT
Preparing for your visit is an important step in partnering with your Hope & Help Medical Provider as an active participant in your overall health care. To make the most of your visit, make a list of questions and concerns you have and share them with your Hope & Help Medical Provider during your appointment. Remember, there's no such thing as a 'silly' or 'unimportant' question. Your health and wellness is what matters most to us at Hope & Help. If you're unsure of what questions to ask your Hope & Help Medical Provider, consider asking yourself these questions:
Before Your Appointment, Ask Yourself
What is the reason for my visit?
What are the symptoms that I am experiencing?
What prescription medications am I taking or should I take and why?
Have I been to the Emergency Room since my last visit?
Am I concerned for my physical safety and or mental health?
In order to schedule an In-Office Visit, click the Appointment button below or call 407.645.2577. Please arrive 30 minutes before your scheduled appointment and bring a photo I.D. and your most current health insurance information. If you're unable to make your scheduled appointment, notify Hope & Help at least 24 hours in advance. A "no show" fee may apply for missed appointments.
Through the convenience of digital technology, you can now receive the same quality face-to-face experience with your Hope & Help Medical Provider wherever it’s convenient for you. At home, work, or anywhere the internet takes you. Telehealth Visits are available to existing patients. If you are a new patient, your Medical Provider may require an In-Office Visit for the initial consultation. To schedule your Telehealth Visit, click the Appointment button below or call 407.645.2577.
URGENT MEDICAL CARE
Hope & Help offers services for routine health and wellness examinations and treatment. If you are experiencing a life-threatening condition, call Emergency Medical Services 9-1-1 or visit a trauma-centered health facility.
Complete these forms prior to your In-Office or Telehealth Visit to expedite your appointment.
ACCEPTED HEALTH INSURANCES
Hope & Help has a growing list of accepted health care insurances. Hope & Help offers both Primary Medical Care and Infectious Disease Care, so it's important to list your Hope & Help Medical Provider as your "Primary Care Provider" with your health care insurance provider to ensure your health services are covered to the fullest extent by your health care insurance provider. If you're unsure about whether or not we can accept your health insurance, give us a call at 407.645.2577.
